Position Highlights

As a Primary Care Physician, you will:

• Provide comprehensive preventive, acute, and chronic disease management
• Care for a diverse adult patient population in Internal Medicine or Family Medicine settings
• Develop long-term patient relationships and coordinate care across specialties
• Utilize evidence-based medicine to promote patient wellness and improved outcomes
• Participate in quality improvement and population health initiatives
• Collaborate with a multidisciplinary team to deliver exceptional patient-centered care

Qualifications

Required

• MD or DO degree
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Eligible for Connecticut and/or New York medical licensure
• Strong commitment to patient-centered care
• Excellent communication and teamwork skills

Preferred

• Interest in quality improvement, value-based care, and population health
• Experience working in a collaborative, team-based environment
