What You’ll Do

• Provide outpatient care focusing on patients aged 55 and older within a PACE program
• Conduct in-clinic and community-based visits, including off-site patient residence visits
• Collaborate with an 11-person interdisciplinary team managing patient care through the EMR system
• Participate in patient assessments, treatment planning, and follow-up visits
• Work with a team to coordinate care in assisted living, SNFs, or private residences
• Manage acute and chronic health conditions specific to the elderly population
• Ensure compliance with all licensing, documentation, and safety standards

What You Need

• Active Oregon medical license and DEA registration
• Board Certified or Truly Board Eligible in Family Medicine or Internal Medicine
• Minimum 2 years of geriatric care experience
• BLS certification
• Ability to drive to patient residences for off-site visits
