What You’ll Do

• Collaborate with residents and the healthcare team to develop and implement patient care plans
• Provide hospice and palliative care services to terminally ill patients
• Document patient assessments, treatment plans, and progress notes accurately
• Participate in care conferences and team meetings
• Support patients and families emotionally and clinically during difficult times
• Adhere to all legal, regulatory, and organizational policies

What You Need

• Current medical license in Virginia
• Certification in Hospice and Palliative Medicine preferred
• At least 1 year of clinical experience in hospice or palliative care (preferred)
• Excellent communication and empathetic patient care skills
• Ability to work collaboratively in a team environment
