BJC Medical Group | Post Acute Care of Missouri & Illinois St. Louis Region

BJC Medical Group is seeking a Board?Certified or Board?Eligible Internal Medicine, Family Medicine, or Geriatric Medicine Physician to join our growing Post Acute Care of Missouri & Illinois team. This role provides care across multiple BJC?affiliated Skilled Nursing Facilities (SNFs) throughout St. Louis County and St. Charles County, with future expansion into Illinois.

About the Role

The Post?Acute Care Physician will provide comprehensive medical care within BJC?affiliated SNFs, functioning as an extension of the Hospital Medicine service line. Facilities operate more like hospital?level environments rather than traditional nursing homes, caring for patients with high acuity and complex medical needs.

Physicians manage new admissions, support APPs with complex cases, and collaborate closely with facility staff, hospitalists, and the broader BJC Medical Group network to ensure continuity of care.

Key Responsibilities

  • Provide admission, follow?up, and transitional care services in SNFs

  • Manage acute and chronic medical conditions in adult and geriatric populations

  • Support APPs with complex cases, family discussions, and care planning

  • Participate in interdisciplinary care conferences and goals?of?care conversations

  • Ensure patients are seen within 72 hours of admission

  • Complete timely documentation in EPIC and facility?specific EMRs

  • Collaborate with hospitalists and primary care physicians to reduce readmissions

  • Maintain availability until 5:00 PM; flexible charting from home

  • Cover 3–4 SNFs weekly (not per day) with APP support

  • Participate in quality initiatives and platform growth

Practice Structure & Support

  • 7 APPs in the practice; at least one APP assigned to the new physician

  • APPs onsite 9:00 AM – 2:00 PM

  • Physicians may complete charting remotely

  • SNFs provide their own labs, ancillary services, and equipment

  • EPIC EMR with remote access + facility?specific systems

  • Strong team?oriented culture with shared coverage and support

  • Providers help one another to maintain work?life balance

Schedule & Call

  • Hours: Monday–Friday, 8:00 AM – 5:00 PM

  • FTE: 1.0 FTE = 36 patient?facing hours

  • Part?time: Not available

  • 4?day workweek: Not available

  • Call:

    • Weekday: Providers take their own call

    • Weekend: 1:4 to 1:6 rotation (approx. once monthly to every 6 weeks)

    • Virtual Call Coverage (VCC) handles non?new admissions

    • Physician handles new admissions after hours

    • Call is not compensated

Patient Volume & Population

  • 12–14 patients per day

  • 30–70 patients per SNF depending on acuity

  • Primarily geriatric patients; occasional 16+

  • High?acuity, medically complex cases

  • Majority Medicare population

Ideal Candidate

  • Board?Certified or Board?Eligible in IM, FM, or Geriatrics

  • Experience in SNF, post?acute, hospital medicine, or urgent care preferred

  • Comfortable managing medically complex patients

  • Strong communication skills for family discussions and care planning

  • Team?oriented and collaborative

  • Must obtain both Missouri and Illinois medical licenses

  • H1B candidates considered; J?1 waivers not available

Role Purpose

The physician will provide professional medical services within the practicing Specialty to the best of physician’s ability through direct patient care and spend additional time as necessary to perform other related duties such as completing medical records, providing MyChart consultations and inbasket management, conducting patient-specific education and collaborating with advanced practice providers and care team members.

 

Responsibilities

  • Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician’s patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician’s hospitalized patients consistent with Medical Staff requirements.
  • Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.
  • Performs and documents medical histories and physicals in the patient’s medical record as required by hospital medical staff bylaws.
  • Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.
  • Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician’s patient panel.
  • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
